This Telehealth Informed Consent describes the nature of telehealth care offered through Womea, its benefits and limitations, and your rights. This is a templated document provided for illustration; the clinician who provides your care will obtain consent as required in your state.

Nature of telehealth

Telehealth uses secure technology to deliver care at a distance. Care is provided by clinicians of Arora Health & Aesthetics, an independent provider group; Womea is a technology platform and does not employ them. To begin hormone therapy, you have a live (synchronous) video or phone visit with a US-licensed clinician, who reviews your information and makes any prescribing decisions. Follow-up care may be provided asynchronously through secure messaging.

Benefits

Telehealth can make evidence-based menopause care more accessible and convenient, allowing review of your symptoms and history without an in-person clinic visit, and ongoing messaging with your care team.

Limitations and risks

Telehealth is not appropriate for every condition. Without an in-person examination, a clinician may have less information than during a traditional visit, and there is a possibility that a condition could go undetected. Technology failures or delays may also occur.

If telehealth is not appropriate for your situation, the clinician may request additional information, recommend an in-person visit, or decline to prescribe.

The clinician makes prescribing decisions

Submitting an assessment or payment does not guarantee a prescription. A licensed clinician independently determines whether treatment is appropriate for you based on your history and the applicable clinical guidelines.

Your rights

Your participation is voluntary, and you may withdraw or decline treatment at any time without affecting your right to future care. Your information is handled as described in our Privacy Policy.

Emergencies

Telehealth is not for emergencies. If you are experiencing a medical emergency, call 911 or go to the nearest emergency room. Do not use messaging for urgent or emergency needs.
